Summary
This study tests a pharmacy-led program for low-income patients with multiple chronic conditions like diabetes, heart disease, and depression. After leaving the hospital, participants get their medications with no copay, delivery to their bedside and home, and help from a pharmacy technician to manage their medicines and follow up with a doctor. The goal is to see if this program helps patients take their medications as prescribed and reduces hospital readmissions over one year.
