New program aims to help low-income patients stick with meds after hospital discharge

NCT ID NCT06374277

First seen Jun 25, 2026 · Last updated Jun 27, 2026 · Updated 1 time

Summary

This study tests a pharmacy-led program for low-income patients with multiple chronic conditions like diabetes, heart disease, and depression. After leaving the hospital, participants get their medications with no copay, delivery to their bedside and home, and help from a pharmacy technician to manage their medicines and follow up with a doctor. The goal is to see if this program helps patients take their medications as prescribed and reduces hospital readmissions over one year.

What this could mean

Our plain-language read of the trial. This is informational only — not medical advice or a prediction.

Active substance

Med AAAction intervention (zero-copay medications, bedside and home delivery, care coordination by pharmacy technicians)

What this could lead to

If it works, this program could help low-income patients with chronic conditions take their medications more consistently after a hospital stay, potentially reducing hospital readmissions and healthcare costs.

What could go wrong

This is a relatively small trial (388 participants) testing a complex intervention, so results may not apply broadly. The program's success depends on many factors, and it may not significantly improve medication adherence or health outcomes.
