Summary

This study tests a web-based tool called gp-multitool.de that helps general practitioners follow guidelines for older patients with multiple chronic conditions. About 659 patients from GP practices in Germany are taking part. The tool lets patients fill out questionnaires online and helps doctors review medications and document care decisions. The main goal is to see if using the tool reduces hospital stays over a year.
