Summary
This study aims to find better ways for doctors and patients to talk about what matters most when facing a serious illness like cancer, heart failure, COPD, or dementia. Researchers will track how often and how early these conversations happen in the last six months of life. The goal is to improve communication so that care matches each patient's wishes.
