Summary

This study tests whether an adapted questionnaire, called the Care Partner Hospital Assessment Tool (CHAT-AD), can help care partners of hospitalized people with Alzheimer's disease or related dementias feel more prepared to provide care after discharge. Participants are unpaid family members or friends caring for a hospitalized adult with dementia. They will complete the questionnaire and other surveys during and after the hospital stay, over a period of 14 days. The goal is to see if the tool improves care partner preparedness and satisfaction with care.
