Summary
This study tests a web-based tool called gp-multitool.de that helps family doctors follow national guidelines for older patients with multiple chronic conditions like diabetes, heart disease, and depression. About 659 patients from various doctor practices in Germany are taking part. The goal is to see if using the tool reduces hospital stays and improves how care is coordinated.
