Putting Patients' priorities first: a new approach to ease the burden of multiple chronic illnesses

This pilot study tests a care model called Patient Priorities Care (PPC) for older African Americans with heart disease plus other chronic conditions like diabetes or COPD. The approach shifts focus from treating each disease separately to aligning care with what matters most to the patient. Researchers will check if this culturally tailored version is acceptable and feasible, and whether it improves patients' care experiences, reduces treatment burden, and enhances communication with clinicians.
