Summary
This study looks at whether having a cancer survivor's regular doctor work closely with their cancer team can help manage other health problems like high blood pressure, diabetes, and high cholesterol. About 800 adults with certain cancers (like breast, prostate, or lung) and at least one of these conditions took part. The goal is to see if better teamwork between doctors leads to better control of these long-term health issues.
