New program aims to slash repeat hospital visits for heart and kidney patients

Summary

This study tests a program called Moxie that sends weekly mail and a mobile app to help people with chronic heart, kidney, or diabetes conditions manage their health after a hospital stay. The goal is to see if it reduces the chance of going back to the hospital. About 9000 adults in Alberta will take part, and some will also get letters to share with their doctors and pharmacists.
