Summary

This study tests a pharmacy-led program to help low-income patients with chronic conditions like diabetes and heart disease take their medications after leaving the hospital. The program provides free medications, bedside delivery, and health coaches to help with follow-up care. About 388 Medicaid or uninsured adults will take part to see if this approach improves medication use and reduces hospital visits.
