Summary
This study tests a program called Med AAAction for low-income patients with multiple chronic conditions like diabetes, heart disease, and depression. The program provides free medications, delivers them to the bedside and then home, and assigns a health coach to help with follow-up care. Researchers will see if this helps patients take their medications as prescribed and reduces hospital readmissions over one year.
