Summary
This study follows overweight and obese adults aged 45-65 to see if a tailored program of counseling, supervised exercise, and digital tools helps them lose weight and improve heart health over 18 months. Participants are compared to a group that only gets general written advice. The goal is to find a cost-effective way to manage weight and reduce diabetes and heart disease risk.
