Health coaches to the rescue: can community workers help patients manage multiple illnesses?

Summary

This study will see if community health workers (CHWs) can help people with two or more chronic conditions like diabetes, high blood pressure, or heart disease better manage their health. About 300 adults who are medically underserved will work with a CHW to create a personal health plan. The goal is to improve their confidence and ability to navigate the healthcare system and take control of their own care.
