Summary
This study tests a program called Transition to Cardiac Rehabilitation (T2CR) for veterans with heart disease who also have other health problems or frailty. The program provides in-person support to help them start and succeed with home-based cardiac rehab. Researchers will compare their physical function, rehab participation, and healthy days at home over one year against those receiving usual care.
