Summary
This study looks at whether giving people with inflammatory arthritis a list of community resources, a community specialist, or a nurse navigator can reduce missed doctor visits. About 100 adults with conditions like rheumatoid arthritis or lupus who have missed appointments in the past year and have social needs (like food or housing help) are taking part. The goal is to find the simplest, most cost-effective way to help people get the care they need.
