NCT ID NCT05592847
First seen Jun 26, 2026 · Last updated Jun 27, 2026 · Updated 1 time
Summary
This study tests whether a nurse navigator program can reduce hospital readmissions in 300 patients who are having hip or knee replacement and also have conditions like heart failure, COPD, or kidney problems. The program provides education and connects patients with their primary care doctors and specialists. Researchers will compare readmission rates and patient satisfaction between those who receive the program and those who do not.
