Summary
This study tests a pharmacy-led program to help low-income patients with multiple chronic conditions take their medications correctly after leaving the hospital. The program provides free medications, bedside delivery, and health coaches to address common barriers like cost and transportation. About 388 Medicaid or uninsured adults will take part, and researchers will track medication use, hospital readmissions, and costs over 12 months.
