New study aims to help Low-Income patients stick to meds after hospital discharge

Summary
This study tests a program to help low-income people with multiple chronic conditions take their medications correctly after leaving the hospital. The program provides free medications, delivers them to the bedside and home, and offers support from pharmacy staff. Researchers will compare this program to usual care in 388 patients to see if it improves medication adherence and reduces hospital readmissions.
