Summary

This completed study tested a health system intervention called ENHANCE in 1,837 adults aged 40+ with at least two chronic conditions (HIV, high blood pressure, diabetes, asthma, or depression). The intervention included a clinical decision support tool for clinicians, health education materials, and home visits from community health workers. The goal was to see if this approach could improve detection of new conditions, treatment changes, and control of existing conditions compared to usual care.
