Summary
This study tests whether using telemedicine (video or phone check-ins) can help doctors better adjust heart failure medications after patients leave the hospital. About 140 adults with heart failure and reduced pumping ability will be split into two groups: one gets usual care, the other gets digital follow-up. The goal is to see if the telemedicine group gets their medicines optimized faster and more safely.
