Summary

This study tests whether an AI-powered health coach, along with a vital sign monitor, can help 300 adults with heart disease, diabetes, or kidney disease better manage their health. Participants will use a chatbot and tracking tools to improve key health numbers and reduce hospital stays over 6 months. The goal is to see if this technology can lower the risk of serious complications like heart attacks or kidney failure.
