Summary

This study looked at two different methods for assigning care coordinators to older adults (65+) who are at risk for heart disease, stroke, or related conditions. The goal was to see which method better prevents emergency room visits and hospital stays. About 400 participants from a Medicare accountable care organization took part, and the study compared a new assignment approach to usual care.
