This study tests whether an online training and peer support platform (Support-t) helps teens with type 1 diabetes prepare for the move from pediatric to adult healthcare. About 200 teens aged 14-16 will either use the platform plus usual care or just usual care. The main goal is to see if the platform helps keep blood sugar levels stable over 18 months.
