Summary
This pilot study will test a new digital health tool designed to help adults aged 40 and older who have two or more long-term health conditions like high blood pressure, diabetes, or depression. The tool includes a system to help doctors make decisions, remote specialist visits, and a mobile app to support patients. About 180 people in India and Nepal will use the tool for three months, and researchers will check if it is easy to use and helpful. The goal is to learn how to improve the tool before testing it in a larger study.
