First seen Jun 26, 2026 · Last updated Jun 27, 2026 · Updated 1 time

Summary

This study tests whether adding a structured self-management program (DECIDE) to standard community health worker support helps 500 Philadelphia residents aged 35-75 better manage heart disease risks and meet their social needs. Participants will either receive the enhanced program or continue with usual services. Researchers will measure improvements in self-care skills and resolution of social needs like housing or food access.

What this could mean

Our plain-language read of the trial. This is informational only — not medical advice or a prediction.

Active substance

community health worker support and a self-management education program called DECIDE

What this could lead to

If it works, this could show that combining self-management training with community health worker help improves heart health and reduces social needs in underserved communities.

What could go wrong

This is a behavioral study, not a drug trial, so results depend on participation and may not apply to other cities. It is also early-stage research, so benefits are uncertain.
