Summary
This study tests a new support program for low-income patients with chronic diseases like diabetes and heart problems after they leave the hospital. It provides free medications, delivers them to the bedside and then to the home, and offers a coach to help with follow-up care. Researchers want to see if this practical help makes it easier for people to take their essential medicines, which could keep them healthier and out of the hospital.
