Summary

This study tests whether a pharmacist-led care pathway can improve blood sugar control in 600 adults with type 2 diabetes who are not meeting their A1C targets. Participants will receive step-by-step guidance from pharmacists using a web-based tool, compared to usual care from their doctor or nurse. The goal is to see if this approach helps people achieve healthier blood sugar levels and reduce the risk of diabetes-related complications.
