Summary
This study is testing a new support package for older adults who have two or more long-term health problems, like high blood pressure, diabetes, or arthritis. The package includes digital tools, personalized guidance, and ongoing support to help manage their health. Researchers will compare this package to standard care for about 394 people over 6 months to see if it reduces hospital readmissions and improves quality of life.
