This trial tests whether a simple saliva test, called MyPhenome, can guide doctors in choosing the most effective anti-obesity medication for each individual. The test sorts people into biological subtypes—like 'hungry brain' or 'hungry gut'—to personalize treatment. About 424 adults with obesity or overweight will be randomly assigned to either test-guided or standard medication selection, and their weight loss will be tracked for up to two years.
