Summary
This study looks at whether a 12-week education and support program can help African American patients with advanced chronic kidney disease learn more about their condition and how to manage it. About 180 hospitalized adults will either receive the program or standard care. The goal is to see if the program improves knowledge about kidney disease, treatment options, and self-care.
