Summary
This study tests a new digital platform that helps hospitals share patient information more reliably when a patient is transferred from one hospital to another. About 1,000 adults being moved to Brigham and Women's Hospital from three other hospitals will take part. The goal is to see if better information sharing reduces medical errors and improves patient safety.
