Summary
This study tests whether giving heart failure patients a one-page checklist of recommended medications helps doctors prescribe them more often. The checklist lists guideline-approved drugs and doses for heart failure with reduced ejection fraction. Researchers will enroll 4000 patients across cardiology clinics to see if this simple tool increases use of these lifesaving medications.
