Summary

This study tests a mobile app that helps heart failure patients track their symptoms, vital signs, and weight. The app shares this information with doctors to improve medication use and quality of life. About 130 adults with heart failure and reduced pumping function will participate. The study focuses on better disease management, not a cure.
