New portal tool aims to cut risky meds for dementia patients

This study tests a program that uses a patient portal to help older adults with mild cognitive impairment or dementia, along with their care partners, reduce medications that may be harmful. Participants receive educational materials and can meet with a pharmacist to review their medicines. The goal is to see if this approach helps people stop unnecessary drugs and better match their treatment to what matters most to them.
