First seen Jun 26, 2026 · Last updated Jun 27, 2026 · Updated 1 time
Summary
This study tests a 6-month digital care program for adults aged 50+ with type 2 diabetes and at least one other chronic condition like high blood pressure or kidney disease. The program combines online education, phone support, a mobile health app, home visits, and personalized care plans. Researchers will track whether it improves disease control, self-management, depression, and quality of life compared to usual care.
What this could mean
Our plain-language read of the trial. This is informational only — not medical advice or a prediction.
Active substance
Digital Care Community Common Good program (behavioral intervention including online and in-person education, telephone care, mHealth platform, home visits, and individualized care plans)
What this could lead to
If successful, this program could provide a practical, community-based approach to improve self-management and quality of life for older adults with diabetes and multiple chronic conditions.
What could go wrong
This is a small, early-stage study (169 participants) focused on behavioral changes, not a drug or cure. Results may not apply to other populations or settings, and improvements may be modest.
