Digital boost for heart recovery: new study tests App-Based support

Summary
This study tests whether a digital support package (DEEPER) helps heart patients maintain health after completing cardiac rehab. About 306 adults with heart disease or heart failure will either get standard care or standard care plus the digital program. The main goal is to see if the digital program improves heart health and mood over 6 months.
