Summary

This study tests whether a digital support program (called DEEPER) helps heart patients maintain their health after completing cardiac rehab. About 306 adults with heart disease or heart failure will be split into two groups: one gets standard follow-up care, the other gets standard care plus the DEEPER digital tools. The main goal is to see if the digital program improves heart health and mood over 6 months.
