New diabetes clinic model aims to help vulnerable patients control blood sugar

This study tested a special diabetes clinic within a primary care setting for patients with poorly controlled type 2 diabetes. The clinic brought together doctors, diabetes educators, nurses, behavioral health providers, and social workers to offer comprehensive care, including continuous glucose monitoring. Researchers enrolled 65 patients and measured changes in blood sugar levels, weight, and how well the program worked. The goal was to see if this team-based approach could improve diabetes management and address social factors like food and housing insecurity.
