New toolkit aims to keep diabetes patients safe after hospital discharge

Summary

This study tests a special toolkit designed to help people with type 2 diabetes who are starting insulin for the first time when they leave the hospital. About 120 English-speaking adults will be randomly assigned to receive the toolkit or usual care. The goal is to see if the toolkit helps keep blood sugar levels stable and reduces dangerous highs and lows after discharge.
