Summary
This trial tests a small sensor placed in the pulmonary artery (the blood vessel from the heart to the lungs) that wirelessly sends daily pressure readings to doctors. The goal is to help people with moderate heart failure (NYHA Class II or III) manage their condition at home and avoid hospital stays. About 1,750 participants will be randomly assigned to get the sensor or standard care, and researchers will track heart failure events and deaths over up to 24 months.
