Summary
This study tested a tool called HealtheRx, which gives African American patients in rural North Carolina a personalized list of local resources for food, housing, transportation, and more. The goal was to see if this information helps patients feel more confident finding and using community resources to manage their heart health. Over 300 participants received the HealtheRx and could also talk to a community resource navigator for extra support.
