This study tested whether giving African American patients in rural eastern North Carolina a personalized list of local resources (like food, housing, and transportation help) could improve their confidence in managing their heart health. Over 300 participants received a 'HealtheRx' resource sheet and could request help from a navigator. The main goal was to see if this approach increased their self-efficacy in finding community resources.
