Summary
This study is creating a large database of 8,000 adults with heart, kidney, or metabolic diseases to track their health over up to 5 years. Researchers will collect medical records, imaging, and lab data to understand how these conditions interact and affect long-term outcomes. The goal is to improve risk prediction and care for people with Cardio-Renal-Metabolic (CKM) syndrome.
