Summary

This study tests whether using a real-time continuous glucose monitor (CGM) can improve blood sugar control and reduce hospital readmissions in people with type 2 diabetes and acute heart failure. Researchers will follow 140 adults during their hospital stay and for three months after discharge, comparing CGM to standard finger-stick testing. The goal is to see if better glucose monitoring can prevent dangerous blood sugar swings and lower the risk of returning to the hospital.
