Summary
This study looks at a new way to give recommended vaccines to adults with chronic diseases or weakened immune systems. Doctors will offer vaccines during hospital visits, admissions, or at discharge. The goal is to see how many patients accept and complete their vaccines, and to make it easier for hospitals and local health services to work together. About 1500 people will take part.
