Summary
This study tests whether a program using community health workers and medication can lower blood pressure and prevent heart failure in 364 adults from Detroit and Cleveland. Participants with high blood pressure or risk factors will receive personalized support and medication. The goal is to see if this approach improves heart health over 12 months.
