Remote Check-Ins may keep kidney Patients' blood pressure in check

Summary

This study looks at whether checking blood pressure at home and sharing readings with a case manager can improve blood pressure control for people with chronic kidney disease who live far from clinics. About 146 adults in northern Alberta will either use home monitoring with usual care or add a case manager who adjusts medications. The goal is to see if the extra support leads to better blood pressure over one year.
