This study tested whether a structured discharge education session using the 'teach-back' method—where patients repeat instructions in their own words—could help heart failure patients better understand their condition and self-care routines. The goal was to reduce 30-day hospital readmissions. The study involved 57 adults with systolic heart failure who were hospitalized at the time.
