Summary

This study tested a technology-based program (T-FAME) that helps heart failure patients and their families manage care at home after a hospital stay. The program uses a smartphone app to support self-care and family involvement, aiming to improve quality of life and reduce hospital readmissions. 270 patients in Hong Kong participated.
