Summary
This study tested a technology-based program (T-FAME) that helps heart failure patients and their families manage care at home after a hospital stay. The program uses a smartphone app to support self-care and family involvement, aiming to improve quality of life and reduce hospital readmissions. 270 patients in Hong Kong participated. The goal was to see if this approach is effective and cost-saving.
