Summary

This study tests whether a 12-week smartphone-assisted cardiac rehabilitation program, led by a case manager, can help heart failure patients exercise more and feel better compared to usual care. About 150 adults with stable heart failure will take part. The goal is to see if the program improves exercise capacity, quality of life, and adherence to exercise.
