Summary

This study tests a nurse-led program that uses home monitoring devices to help older adults with conditions like heart failure, diabetes, and depression manage their health after leaving the hospital. The goal is to improve care coordination and reduce hospital readmissions. About 350 participants will use telemonitoring equipment and receive support from a nurse or nurse practitioner.
