Summary
This study tests a new type of clinic that checks people with prediabetes and type 2 diabetes for several health problems at once, including fatty liver disease, sleep apnea, and heart failure. About 400 adults will fill out questionnaires and use simple devices to find these hidden conditions. The goal is to see if this approach can catch problems early and improve care.
