New hospital plan aims to fix heart failure care gaps for minority patients

This study tested a new care plan for heart failure patients admitted to general medicine, who are often Black or Latinx and receive less specialized care. The plan included standard treatments, cardiology checkups, and help with transportation and follow-up. The goal was to see if this approach could improve follow-up care and reduce hospital readmissions.
